This question seems to imply that RSTP can operate with PVST+. The Odom book says it can't.

You're right about this. Page 88 of the Cisco Press: CCNA ICND2 Guide by Wendell Odom, gives a table in the middle of the page. The first row of the table has the option PVST+ and in the column "Supports RSTP", the answer is "No". In thinking about this, isn't that why PVRST and MIST were created?
